These terms govern PSC Dx's Customer Care SMS messaging program. By providing verbal consent to a PSC Dx contact center agent, you agree to receive text messages under the terms described below. Please read this page carefully.

1. Program Description

PSC Diagnostics LLC, doing business as PSC Dx ("we," "us"), operates a Customer Care SMS messaging program (the "Program") to support referring healthcare provider offices that have submitted diagnostic test orders to our laboratory.

Through the Program, our contact center agents send and receive text messages with administrative and clinical staff at provider offices regarding open test orders, specimen requirements, results portal questions, and related account follow-ups.

2. How You Opt In

Enrollment in the Program is by verbal consent only. You will be enrolled only if a PSC Dx contact center agent speaks with you by phone, reads the following consent script, and you verbally agree:

Agent script (read verbatim):

"To follow up faster on this order, may we send text message updates to this phone number? You can reply STOP at any time to opt out, and message and data rates may apply. Do you consent?"

If you say yes, the agent will document the consent in our system — capturing the date, time, your name, your provider office, the agent who took consent, and your phone number — and you will receive a welcome SMS confirming enrollment.

PSC Dx does not enroll recipients through web forms, paper forms, or keyword-based opt-in for this Program.

3. Message Frequency

Message frequency varies based on the number of open inquiries associated with your provider office. A typical inquiry generates 1 to 5 text messages between you and a PSC Dx agent.

We do not send marketing or promotional content through this Program.

4. Message and Data Rates

Message and data rates may apply, based on your wireless carrier plan. PSC Dx does not charge for messages sent through this Program.

5. How to Opt Out

You can opt out of the Program at any time by replying STOP to any message from PSC Dx. You will receive a final confirmation message and no further messages will be sent.

To resubscribe, reply START or contact our office directly.

7. Supported Carriers

This Program is compatible with major U.S. wireless carriers: AT&T, T-Mobile, Verizon Wireless, Sprint, U.S. Cellular, and others. Carriers are not liable for delayed or undelivered messages.

8. Privacy

Information collected for this Program is handled in accordance with our Privacy Policy at psc-dx.com/privacy.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All opt-in data and consent records are excluded from any third-party data sharing.
